Assisted Living - Assists eligible seniors living at home to maintain their independence with an optimal level of health and well-being.

Who can use Assisted Living Services?

The transportation service is available to people who meet these criteria:

  • Must be 65 years or older or have a geriatric profile
  • Must meet eligiblity criteria as determined by an assessment process with LHIN Home and Community Care
  • Must reside in a designated geographic service area
  • Must be able to remain safely at home between visits
  • Must be able to direct their own care or have a substitue decision maker to direct care
  • May live alone or with a caregiver
  • Must not pose a risk to themselves or others
  • Must be medically stable

What type of Assisted Living Services are available?

  • Personal care
  • Light housekeeping
  • Medication prompts and compliance
  • Exercises
  • Meal preparation
  • Laundry & linen change
  • Security checks
  • Emergency Response System installed in the home
  • 24-hour urgent response

How do I apply?

To apply, call Home Support at 613-756-2772 or drop by our office.

A referral must be sent by one of the following:

  • Pyhsician
  • Hospital Discharge Planner
  • Community Support Service Agency
  • LHIN Home and Community Care Case Manager
  • Caregiver or self

Admission to Assisted Living Service is completed by our community partner LHIN Home and Community Care through their Central Intake Process. Once the client is assessed, the Community Support Service Agency in the geographic area will contact the client to discuss the services. If the program is full, the senior will be put on a waiting list.

The care plan for the individual is developed by the senior and the Care Coordinator, to best meet the needs and maintain the independence of the individual.

Is there a fee for Assisted Living Services?

There is no fee for seniors who are eligible to receive Assisted Living Services. The cost is funded through the Champlain Local Health Integration Network and Ministry of Health and Long Term Care.
